Klinefelter syndrome
Also called: 47,XXY
Overview
Board takeaway: Assess taurodont root and pulp anatomy before pulp therapy or extraction, and adapt explanations to language and learning needs.
How does Klinefelter syndrome affect dental management?
- Before pulp therapy or extraction: obtain radiographs. Taurodont pulps are large, furcations are deep, and canal orifices may lie near the apex.
- Pulp therapy: deep canal orifices can be difficult to locate, making treatment technically harder.
- Extraction: review actual root number, shape, divergence, and neighboring structures. Taurodontism does not reliably make an extraction easy.
- Communication and history: tailor explanations to language and learning needs; review endocrine treatment.

What is Klinefelter syndrome?
A sex chromosome condition in males involving an additional X chromosome, often associated with testicular insufficiency and variable learning or language difficulties.
What are the main systemic features of Klinefelter syndrome?
- Genetics: 47,XXY; the most common sex chromosome aneuploidy in males (~1 in 600); nondisjunction, risk rises with maternal age
- Endocrine features: Tall stature with long legs, small firm testes, hypergonadotropic hypogonadism, gynecomastia, infertility
- Development: Speech, language and learning difficulties; often diagnosed at puberty or not at all
- Treatment: Endocrinology may use testosterone replacement for hypogonadism; timing and need depend on the individual
What oral findings occur in Klinefelter syndrome?
- Taurodontism (enlarged pulp chambers, apically displaced furcations), mostly in molars
- Tooth dimensions and root morphology can vary; assess the individual radiograph rather than assuming all roots are long
For Klinefelter syndrome, which other conditions are associated with taurodontism?
- Down syndrome
- Ectodermal dysplasia
- Tricho-dento-osseous syndrome
- Amelogenesis imperfecta: hypomaturation–hypoplastic type
- Other X chromosome aneuploidies
