How does Immune thrombocytopenia affect dental management?
Before invasive care: Hematology consult and current platelet count before any invasive care
Local anesthesia: Agree on hemostatic support before a deep inferior alveolar nerve block when thrombocytopenia or bleeding risk is significant; a low count does not automatically mean platelet transfusion.
Hemostasis: Atraumatic technique, local hemostatics, sutures, tranexamic acid
Pain control: acetaminophen is preferred; avoid aspirin and NSAIDs.
Steroid history: review recent and prolonged courses. Ask about established or suspected adrenal suppression before stressful treatment; steroid exposure alone does not establish a stress-dose regimen.

Active mucosal bleeding or hemorrhagic bullae warrants prompt assessment. An elective extraction requires current platelet trend, bleeding history, and a hematology-directed procedural plan; transfusion is not automatic.
