How does Sickle cell disease affect dental management?
Medical coordination: Hematology consult; check CBC; transfusion may be needed before GA
Antibiotics: AAPD lists functional asplenia as a situation where prophylaxis may be considered. Decide procedure-specific coverage with hematology; daily penicillin prophylaxis and treatment of an established odontogenic infection are separate decisions.
Local anesthesia: Local anesthetic with epinephrine is acceptable in usual doses
Pain: Acetaminophen or opioids for pain; use NSAIDs cautiously given renal risk and coordinate with the physician
Anesthesia setting: choose sedation/GA and setting with hematology and anesthesia according to disease severity, prior acute chest events, organ function, and procedure.
Hospital planning: significant disease or complex care may require a hospital plan for oxygenation, hydration, temperature, pain, and recovery.
